Scheduling instructions for Remote Educational Consults

If you’ve never had an office visit and you’d like a remote educational consult, these are not doctor-patient visits, and Dr. Davis can only provide his opinion and other information. He can’t contribute to your medical care, order labs, prescribe medicines, or recommend or supervise other interventions unless you’ve seen him in-office in Oregon. A Consultation Disclaimer Form must be filled out and returned by e-mail or fax before a remote consultation begins.

In the “reason for visit” section, please include your Skype ID or phone number as necessary. You’ll receive an email with a link to fill out forms – please look out for it!

Scheduling instructions for existing patients

If you are an existing patient of Dr. Davis, and you’d like to schedule a phone-based follow up (not covered by insurance), you can do so below.
